Course Description: This course explores occupational therapy’s role in supporting clients with co-existing mental and physical health conditions to safely transition from institutions and reside in the community. We will discuss challenges that are faced during these transitions as well as what is currently being done in practice to overcome these barriers and facilitate safe, client-centered transitions.
